People seeking health care in Australia can now register for a personally controlled electronic health (eHealth) record. Healthcare provider organisations can also register to participate in the eHealth record system.
The resources below will help you learn about the eHealth record system, and how having an eHealth record will, over time, help to deliver better, more effective and efficient health outcomes for all Australians. eHealth and mental health scenarios
The following video scenarios have been developed to help consumers understand how the eHealth record system can be used in association with mental health management and recovery. The videos were developed in consultation with mental health providers and consumers and features actors in the various roles. Statutory declaration template
If you wish to apply to register someone else on the basis that you are an appropriate person to act on their behalf, you must provide a statutory declaration with information supporting your application (see pages 15-17 of the eHealth registration booklet for further details).
A statutory declaration is a written statement that allows a person to declare something to be true. A Commonwealth statutory declaration must be signed in the presence of an authorised witness. An authorised witness can be someone such as a doctor, teacher, pharmacist, bank officer or a Justice of the Peace.
